Postnatal Sex: When Is It Safe to Resume After Childbirth, Why Does It Hurt, and What Can Help with Pain, Dryness, Low Libido and Vaginal Laxity?

Most doctors recommend waiting 4–6 weeks before having sex after giving birth . Pain, dryness and changes in sensation are common during the first 6–12 weeks . If the symptoms last for more than 3 months (particularly painful sexual intercourse, vaginal laxity or loss of sexual desire) may indicate conditions that require clinical assessment rather than further delay.

 

When can you have sex after giving birth?

The standard clinical recommendation is to resume sexual intercourse after waiting 4–6 weeks following a vaginal birth and 6–8 weeks following a caesarean section . This timeline is based on ACOG and NHS guidelines: it allows for the cervix to close, lochia to cease, and acute tissue trauma to heal (American College of Obstetricians and Gynaecologists, Committee Opinion No. 736, 2018; NHS, nhs.uk/pregnancy) .

For sexual intercourse to be physically safe, three important stages must be completed. Women’s sexual health :

Milestone 1

Lohia (postnatal uterine discharge) stops completely; usually after 4–6 weeks.

Milestone 2

The cervix closes; this occurs at the same time as the end of the lochia.

Milestone 3

Healing is visible on the surface of the episiotomy or perineal tears; internal healing, however, continues over the following weeks.

A comparison of vaginal delivery and caesarean section. Recovery following a vaginal birth focuses on the perineum, the pelvic floor and the vaginal mucosa. Following a caesarean section, however, it is essential to preserve the structural integrity of the abdominal and uterine incisions; nevertheless, hormonal changes affect the vaginal mucosa regardless of the mode of delivery.

What happens if you don’t wait six weeks after giving birth?

Having sexual intercourse before the end of the postpartum period carries a clear and specific risk of infection . As long as the cervix remains partially open, bacteria in the vaginal canal can ascend into the uterine cavity; this increases the risk of endometritis and pelvic inflammatory disease. Unhealed perineal tears may reopen due to friction. In women with connective tissue sensitivity, resuming sexual intercourse too soon may exacerbate underlying pelvic organ prolapse. (ACOG Practice Bulletin, 2018).

Risk is proportional to the timing: sexual intercourse before the third week carries a significantly higher risk than sexual intercourse in the fifth week. It cannot be said that sexual intercourse is completely prohibited before the six weeks following childbirth; this is a threshold based on biological indicators, not a calendar date. The safest rule is to wait until the postnatal discharge has stopped, the tears have healed and any discomfort has subsided.

Why sex feels different after giving birth, and when this is not normal

For most women, sex life feels different after childbirth. Physiological changes (decreased oestrogen levels, changes in pelvic anatomy, changes in the pelvic floor muscles) are almost universal in the early postnatal period. What varies is the severity of these symptoms and whether they resolve on their own. Symptoms that gradually improve are part of the normal recovery process. Symptoms that do not improve or worsen after three months require clinical assessment.

Vaginal Looseness: Is There a Sense of Discomfort During Sexual Intercourse After Childbirth?

in the following 6 months, in 34–45 per cent of women Vaginal laxity has been reported; this condition is caused by the stretching of the pelvic floor muscles and fascial support structures during childbirth.

During vaginal delivery, the pelvic floor muscles, fascial supports and levator ani complex are stretched significantly ; it sometimes tears. The vaginal canal may remain wider for months compared to before pregnancy. Most articles on postnatal sex life treat this as a problem of muscle weakness that can be resolved with Kegel exercises.

That is only half the story. Kegel exercises strengthen the pelvic floor muscles. However, they do not restore fascial integrity. When the layer of connective tissue that anchors the vaginal walls loses its tensile strength, this constitutes structural fascial laxity; this is a separate issue requiring treatment at the tissue level, not merely muscle rehabilitation.

Postnatal Dyspareunia: The Pain Is Not Just Muscle Pain

Painful sexual intercourse (dyspareunia) lasting longer than three months after childbirth affects 17–35 per cent of women and when the underlying cause is vaginal atrophy or the formation of scar tissue due to oestrogen deficiency, the condition will not resolve simply by waiting. (Buhling KJ et al., ‘Prevalence and risk factors of postpartum dyspareunia’, Arch Gynecol Obstet. 2006;274(1):17–24. PMID: 16450103).

The reasons listed below are divided into four different categories.

Wound tissue formation

Inflexible remodelling in the areas of an episiotomy or tear repair creates a hard band that causes sharp, localised pain during penetration.

Postpartum Genitourinary Syndrome

Long-term breastfeeding suppresses oestrogen production and leads to thinning of the vaginal mucosa, reduced lubrication, an increase in vaginal pH and increased tissue fragility; this condition shares the same pathophysiology as GSM in the menopause, but is observed in women in their 20s and 30s following childbirth. Most postnatal resources do not name or explain this condition.

Pelvic floor hypertonia

It is a paradoxical condition in which the pelvic floor muscles are not weak but are excessively tense. This causes painful muscle resistance during sexual intercourse. It is the exact opposite of pelvic floor weakness and is treated differently; physiotherapy is used instead of energy-based devices.

Postnatal hormonal fluctuations can reactivate previously dormant endometriosis lesions in women with a genetic predisposition.

Loss of Sexual Desire After Childbirth: How Long Does It Normally Last?

During breastfeeding, prolactin secretion suppresses oestrogen and testosterone, which results in a clinically significant reduction in libido; This physiological effect usually reverses within 3–6 months after weaning in most women.

Prolactin (produced during breastfeeding) directly suppresses oestrogen and testosterone. In the first six months after birth, testosterone levels fall by 40–50 per cent. (Alder EM et al., Br J Psychiatry. 1986;148(1):74–79. PMID: 3697573). A low or absent sex drive during the breastfeeding period is not a relationship or psychological issue, but a predictable physiological response.

Clinical threshold: It is normal for sexual desire not to return 12 months after weaning It is not a postnatal adjustment period. The absence of sexual desire 24 months after the baby’s birth is not a continuing adjustment but indicates a permanent hormonal or metabolic problem.

What is the ‘three-month rule’ after giving birth? When should you stop waiting and seek help?

The three-month rule is not a cultural waiting period, but a clinical threshold. Within the first three months after childbirth, most sexual symptoms (pain, dryness, loss of sensation, low libido) fall within the expected range of recovery. If symptoms persist after three months, this indicates an underlying condition requiring targeted treatment; it is not advisable to continue waiting.

Be patient: the adjustment period following a natural birth:

What is Postnatal Vaginal Rejuvenation?

When can you have sex after giving birth?

Most clinical guidelines recommend waiting 4–6 weeks after a vaginal birth and 6–8 weeks after a caesarean section. The minimum biological requirements are that lochia has completely ceased and that any episiotomy or perineal tear has healed superficially. These are the physical indicators of readiness; the calendar date is of secondary importance. (ACOG Committee Opinion No. 736, 2018; NHS, nhs.uk)

Does sex feel any different after giving birth, or does it feel more comfortable?

Vaginal laxity is observed in 34–45 per cent of women in the sixth month after childbirth. What happens if you have sex before the first 6 weeks after giving birth?

Before lochia ceases and the cervix closes (typically 4–6 weeks), bacteria may ascend from the vaginal canal into the uterine cavity, increasing the risk of endometritis and pelvic inflammatory disease. Unhealed perineal tears may reopen due to friction. For patients who have had a caesarean section, the integrity of the uterine incision is an additional consideration. Sexual intercourse in the 3rd–4th week after childbirth carries a significantly higher risk of infection than sexual intercourse in the 5th–6th week.

When does sexual desire return after giving birth?

Prolactin suppresses oestrogen and testosterone throughout the breastfeeding period. Testosterone levels fall by 40–50 per cent in the first six months after childbirth. A low or absent libido is a normal physiological condition for breastfeeding women. Libido usually returns to normal within 3–6 months after weaning. If libido has not returned to normal 12 months after stopping breastfeeding (including the absence of sexual desire 24 months after the baby’s birth), this constitutes a clinical finding requiring hormonal assessment.

Can vaginal rejuvenation help after childbirth?

Yes, when indicated for painful sexual intercourse following childbirth, vaginal laxity or postpartum genitourinary syndrome, vaginal rejuvenation is a medical procedure rather than an aesthetic choice. Is it safe to have sex 3 or 4 weeks after giving birth?

No. In the 3rd or 4th week after giving birth, lochia is still present in most women and the cervix has not yet closed completely. This period increases the risk of infection; the partially open cervix provides a direct route for bacteria to travel upwards. Some women, particularly those who have had an uncomplicated vaginal birth without tearing, feel physically recovered by the fourth week. However, physical comfort at this stage does not equate to biological safety.

Are Longevity Clinics Worth the Cost?

Longevity clinics are good value for money when they offer physician-led biological age testing, genomic analysis and longitudinal follow-up; the early detection of cardiovascular, metabolic and oncological risks can prevent much more expensive interventions in the future. However, they are not worth the cost when they offer unproven treatments without a validated initial diagnostic assessment.

Three reasons why longevity clinics are worth the cost

1. Early detection offers measurable financial and health benefits. A significant study published in JAMA Network Open has shown that a high level of cardiorespiratory fitness (an important marker of longevity) is associated with a significantly lower risk of long-term mortality, including from cardiovascular disease and cancer. Identifying the risk at the age of 45 is far less costly than treating a preventable heart attack at the age of 60.

2. Biological age is a better predictor of health outcomes than chronological age. If, chronologically speaking, you are 52 years old but are ageing at 80 per cent of the normal rate as measured by DunedinPACE, your risk profile is genuinely different from that of someone ageing at 120 per cent of the normal rate. This information is actionable. It is not simply your date of birth.

3. Genomic data shapes lifelong protection. Whole-genome analysis identifies variants associated with cardiovascular disease, metabolic disorders, neurodegenerative risk and cancer susceptibility decades before symptoms appear. A single comprehensive genomic result, interpreted by a doctor, can reshape the entire approach to preventive medicine for the remainder of a patient’s life.

Two reasons why longevity clinics might not be worth the cost

1. Many clinics sell treatments that lack scientific evidence. A study conducted by MIT Technology Review in 2025 across 82 longevity clinics worldwide revealed that more than a third of these clinics offer stem cell treatments for which there is no clinical evidence that they extend lifespan. Patients who pay €20,000 for an unverified stem cell infusion without an initial genomic or biomarker assessment are not investing in longevity medicine; they are buying expensive hope.

2. Even good data becomes useless without a doctor’s interpretation. Algorithmic reports that claim to diagnose your biological age across 11 organs using ‘131 parameters’ without a doctor’s review of the results do not constitute clinical medicine. These are software products presented in a medical guise. Without context and clinical judgement, abnormal results can lead to unnecessary anxiety or, worse still, a false sense of security.

1. NAD+ Precursors (NMN and NR)

NAD+ (nicotinamide adenine dinucleotide) levels decline with age and are associated with a reduction in cellular energy production and DNA repair capacity. Dr David Sinclair (Harvard) has shared his own supplement regimen with the public: He takes 1 g of NMN and 1 g of resveratrol daily, mixed into yoghurt; to this he adds vitamin D, vitamin K2, 1 g of berberine, a low-dose aspirin (81 mg) and a statin he has been taking since his twenties. Dr Peter Attia, on the other hand, takes a high-dose Omega-3 supplement containing 2 to 3 g of EPA/DHA daily, 5 g of creatine, a combination of various forms of magnesium, and ashwagandha before bed; and aims to maintain his blood Vitamin D levels between 40 and 60 ng/ml rather than taking a fixed dose. Joe Rogan has mentioned that he takes approximately 5,000 IU of Vitamin D daily, following Dr Rhonda Patrick’s advice, and has regularly featured longevity researchers such as Sinclair, Attia, Patrick and Andrew Huberman on his podcast; however, he has not publicly committed to a single, fixed personal supplementation protocol.
